Our Waivers and Codes of Conduct are now part of our online registration process. You can view these documents below.
Prior to registering, you will need to download the Player Medical form, fill it out and have a digital copy ready to upload with the online registration.

Player Media Release Form
BCRA Waiver and Release of Liability Form - Children will not be able to participate on ice without the waiver form.
